Medical History

• Bad reaction to local anaesthetic (probably due to adrenaline in anaesthetic)
• Allergic to pencillin
• Used to smoke cigarettes (gave up 7 years)

Family and Social History

• Born in Melbourne. Married. Two brothers: younger brother recently diagnosed with cancer. He has just finished chemotherapy treatment.
• Two children, the oldest having trouble at school.
• Regularly attends dental clinic for routine care. Attends with children.
